Make online survey with possibility to parse out data fields?

I want to make an online survey, it can be hosted by some online survey platform, like surveymonkey, or don't know what. The only criteria there should be, is the possibility to parse out the data fields that the user entered... I want to put the results together in an SQL Table like in SQL Server or in a MySQL database table...

and I want to make it as easy for me to read out the fields as possible.. is there some online survey platform that offers this? e.g read it out with some tools, or maybe extract and download the results as a .csv file or json file or similar?
